Insurance and billing

Clear answers, before you sit down.

Medicare, Kaiser with an approved referral, and selected PPO plans are accepted. Coverage and referral requirements are verified before scheduling.

Generally accepted

Medicare and Select PPO Plans

  • Medicare with most supplemental plans
  • Aetna PPO
  • Blue Cross PPO
  • Blue Shield PPO
  • Out-of-state Blue Cross / Blue Shield PPO plans

Acceptance depends on your exact plan and current network eligibility. Please call our office to verify coverage before your appointment.

Referral required

Plans Requiring a Referral

  • Kaiser — a referral is required for the visit
  • LiveWell IPA — a referral is required for each visit

The required referral must be received and verified before an appointment can be confirmed.

Verify before scheduling

Medicare Advantage PPO Plans

Medicare Advantage PPO plans must be reviewed with our office before scheduling. These are primary insurance plans, not Medicare supplemental plans, and may have more restrictive coverage and payment requirements.

Not accepted

Plans Not Accepted In-Network

  • Medi-Cal
  • Molina Healthcare
  • Inland Empire Health Plan (IEHP)
  • All HMO plans
  • HMO plans with a Point-of-Service (POS) option
  • Oasis IPA

Important: PSUMG does not accept HMO plans, including plans with a Point-of-Service option.

Out of network

Out-of-Network Insurance Plans

PSUMG is currently out of network with:

  • UnitedHealthcare PPO when used as the primary insurance
  • Humana
  • Cigna
  • Health Net

Patients with these plans may still request an appointment, but they may have a higher deductible, coinsurance, copayment or other out-of-pocket costs. Out-of-network benefits should be confirmed before treatment.

Important Insurance Notice

Insurance participation and individual benefits can change. PSUMG verifies eligibility, referrals and expected patient responsibility before scheduling.

Acceptance of an insurance plan does not mean that every procedure is covered. Benefits are confirmed for each service before it is scheduled.

Patients are responsible for confirming their benefits and for charges not paid by their insurance plan.
