Notice of Privacy Practices

We are required by law to protect the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

• Treatment — to provide, coordinate and manage your urologic care, including sharing information with other clinicians involved in your treatment.
• Payment — to bill and obtain payment from you, your health plan or another payer.
• Health care operations — for quality review, staff training, scheduling, and administration of the practice.
• As required by law — including public health reporting, health oversight activities, and lawful requests from courts or law enforcement.

• Request access to and a copy of your medical records.
• Request a correction to information you believe is inaccurate or incomplete.
• Request confidential communications at a particular address or phone number.
• Request restrictions on certain uses and disclosures.
• Receive an accounting of certain disclosures of your information.
• Receive a paper copy of this notice on request.
• File a complaint if you believe your privacy rights have been violated.

You may file a complaint with the practice or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be penalized or retaliated against for filing a complaint.

Privacy questions and requests may be made by calling the office at 760-320-6005 and asking to speak with the staff member responsible for privacy matters.

We may change this notice and make the revised notice effective for health information we already have as well as information we receive in the future. The current notice will be posted here and available at the office.
